Structured Health Progression System™
Effective Date: February 12, 2026
1. Scope & Acceptance
These Terms of Service (“Terms”) govern access to and participation in the CoreMD Structured Health Progression System™ (“the System”) operated by CoreMD Clinic (“CoreMD,” “the Clinic,” “we,” or “us”).
By accessing or participating in the System, you acknowledge that you have read, understood, and agree to these Terms.
Participation in the System is voluntary and does not create a contractual entitlement to medical services.
2. Nature of Services
CoreMD Clinic provides physician-led medical services in accordance with applicable laws and professional standards in Ontario, Canada.
The Structured Health Progression System™ is an engagement and governance framework designed to support:

Continuity of care

Structured monitoring
-
Educational reinforcement
-
Defined reassessment intervals
-
Patient engagement within established care pathways
The System is not a financial product, insurance plan, subscription entitlement, or credit mechanism.
3. Physician Independence
All medical decisions are made independently by a licensed physician based on:

Clinical appropriateness
-
Professional judgment
-
Patient safety considerations
-
Applicable standards of practice
Participation in the System:

Does not guarantee treatment
-
Does not obligate prescribing
-
Does not influence clinical recommendations
-
Does not create preferential access to care
Clinical judgment supersedes all progression metrics or engagement indicators.
4. CoreMD Points
CoreMD Points are internal engagement markers within the Structured Health Progression System™.
CoreMD Points:

Hold no monetary value
-
Are non-transferable
-
Cannot be redeemed for cash
-
Cannot be applied toward prescription medications
-
Cannot be used to reduce insured or medically necessary services
-
Do not influence eligibility for treatment
Points are awarded solely in recognition of structured participation activities.
CoreMD reserves the right to modify or discontinue the Points framework in order to maintain regulatory alignment and governance integrity.
5. Eligibility & Participation
Participation in the System requires:

Registration as a patient of CoreMD Clinic

Completion of a physician-led assessment
-
Ongoing compliance with recommended follow-up parameters
CoreMD may suspend or terminate participation where required to protect clinical integrity, patient safety, or regulatory compliance.
6. No Guarantee of Outcomes
The Clinic does not guarantee medical outcomes, clinical improvement, or therapeutic success.
Health outcomes depend on numerous variables including:

Individual physiology

Treatment adherence
-
Comorbid conditions
-
External factors beyond clinical control
The System is designed to support structured care — not to guarantee results.
7. Educational Content
Educational materials provided through the System, including digital modules or written resources, are for informational purposes only.
They:

Do not replace individualized medical advice

Must be interpreted within the context of physician consultation
-
Do not create a physician-patient relationship independent of formal clinical assessment
8. Privacy & Data Governance
CoreMD Clinic handles personal and health information in accordance with:

Ontario’s Personal Health Information Protection Act (PHIPA)

Applicable federal privacy legislation
Engagement data generated within the System is used solely for care coordination, internal monitoring, and quality improvement.
CoreMD does not sell or commercialize personal health information.
9. Regulatory Compliance
CoreMD Clinic operates within Ontario and adheres to:

Standards established by the College of Physicians and Surgeons of Ontario

Ethical and medico-legal guidance supported by the Canadian Medical Protective Association

Applicable provincial and federal healthcare legislation
Nothing within the System is intended to induce medically unnecessary services or circumvent professional standards.
10. Modifications
CoreMD Clinic reserves the right to modify, suspend, or discontinue any aspect of:

-
CoreMD Points
-
Engagement criteria
-
Digital access components
Such modifications may occur to maintain regulatory compliance, clinical governance standards, or operational integrity.
11. Limitation of Liability
The Structured Health Progression System™ is an engagement framework and does not constitute a guarantee of care delivery or clinical outcome.
CoreMD Clinic’s liability is limited to the extent permitted under applicable Ontario law.
12. Governing Law
These Terms are governed by the laws of the Province of Ontario and the applicable federal laws of Canada.
Any disputes shall be resolved within the jurisdiction of Ontario courts.

CoreMD Clinic delivers structured, physician-governed care through a defined progression framework designed to reinforce accountability, continuity, and responsible medical oversight.
The Structured Health Progression System™ exists to support clinical integrity — not to replace it.
