CoreMD Points are awarded in recognition of structured engagement within physician-guided care pathways.

Points are not earned for purchasing services, nor are they tied to medical necessity.

They acknowledge participation in defined components of the CoreMD Structured Health Progression System™.

Structured Participation Activities

CoreMD Points may be awarded for engagement activities such as:

1️⃣ Completion of a Physician-Led Initial Assessment

Recognition of structured entry into a defined care pathway following formal medical evaluation.

2️⃣ Attendance at Scheduled Follow-Up Appointments

Acknowledging continuity and adherence to recommended reassessment intervals.

3️⃣ Completion of Designated Educational Modules

Participation in physician-approved educational content designed to reinforce understanding of one’s care pathway.

4️⃣ Submission of Structured Patient-Reported Updates

Providing clinically relevant updates within scheduled monitoring intervals, where appropriate.

5️⃣ Ongoing Participation in Defined Care Pathways

Demonstrated adherence to the structured framework established during physician consultation.


1. Completing Medical Consultations

Earn 50 Points

Patients receive 50 CoreMD Points for completing a scheduled medical consultation within an active CoreMD program.

This includes:

• Initial structured consultation

• Program enrollment visits

• Scheduled medical review appointments

Points are awarded after appointment completion.


2. Attending Follow-Up Appointments

Earn 50 Points

Continuity of care is essential for safe and effective outcomes.

Patients receive 50 CoreMD Points for attending scheduled follow-up appointments within their structured program.

Points are issued once the follow-up visit has been completed.


3. Completing Required Lab Work On Time

Earn 10 Points

Timely laboratory monitoring is an important part of evidence-based care.

Patients receive 10 CoreMD Points when required lab testing is completed within the recommended timeframe established during their medical visit.

Points are issued after confirmation of completed laboratory reporting.


4. Participating in Educational Modules

Earn 30 Points

Education strengthens long-term outcomes.

Patients receive 30 CoreMD Points for completing designated CoreMD educational modules through the CoreMD Learning Portal (LearnDash).

Eligible modules are clearly identified within your dashboard.


5. Completing Program Milestones

Earn 40 Points

Patients receive 40 CoreMD Points upon achieving predefined program milestones.

Examples may include:

• Completion of structured onboarding pathway

• 3-month progress review

• 6-month program retention milestone

• Completion of compliance check-ins

Milestones are defined within each CoreMD program and may vary by care pathway.


6. Submitting Program Feedback

Earn 10 Points

Patients receive 10 CoreMD Points for submitting structured feedback surveys when invited.

Feedback supports quality improvement and service optimization.

CoreMD Structured Health Progression System™

CoreMD Points are awarded exclusively in recognition of structured patient engagement within defined care pathways operating under the CoreMD Structured Health Progression System™.

CoreMD Points:

  • Do not influence prescribing decisions
  • Do not affect eligibility for treatment
  • Do not alter clinical recommendations
  • Are not contingent upon acceptance of any specific therapy
  • Do not provide preferential or priority access to care
  • Cannot be applied toward prescription medications or insured medical services
  • Hold no monetary or transferable value

All medical decisions are made independently by the treating physician and are guided solely by clinical appropriateness, professional judgment, and patient safety.

Clinical care at CoreMD Clinic is delivered in accordance with:

  • Standards established by the College of Physicians and Surgeons of Ontario
  • Ethical and medico-legal guidance supported by the Canadian Medical Protective Association
  • Applicable provincial and federal healthcare legislation within Ontario

Participation in the CoreMD Structured Health Progression System™ is entirely voluntary and does not create entitlement to medical services.


Operational Transparency

CoreMD Points are credited automatically following completion of qualifying engagement activities within an active care pathway under the CoreMD Structured Health Progression System™.

CoreMD Points:

  • Are issued solely upon verified completion of designated engagement activities
  • Hold no monetary or cash-equivalent value
  • Are non-transferable and may not be assigned to another individual
  • Are recognized only within the context of an active CoreMD program
  • Remain subject to periodic review and administrative adjustment in alignment with program governance updates

CoreMD Clinic reserves the right to modify point allocations, engagement criteria, or structural elements of the System as necessary to maintain regulatory alignment, clinical integrity, and program coherence.
